Light and angle play large roles in this image as well. We normally look down on pumpkins so this image shot slightly upwards throws off our normal perceptions. Then the lighting inside the pumpkin is bright enough to outline the edges of the design nicely and the design itself has a flame look to it so that it has continuity with the interior light. Here too, the facial features are placed by the rule of thirds and the lone subject is mostly centered (although it is a bit offset to the right).
